1) First of all open the Messaging App & Create a new message.
2) Write "PORT 94XXXXXXXX" and send it to 1900. Change 94XXXXXXXX with Your no. Which you wanna port. (See Screen Shot Below↓)
3) After sending the message, you will receive a message from 1901 to verify the port out request.

After a few minutes/hours, you will get a
Unique Port Code (UPC). Keep the message safe it will be required in future. (The UPC is valid for 15 days only, so once you receive it, port to your desired operator within 15 days)
5) Now walk into Jio Store with documents, and Jio offer & UPC Code.
